Job Summary
Provide pediatric physical therapy services in-home and in community settings to children with physical, neurological, cognitive, and/or social-emotional needs. Implement treatment plans, perform evaluations, set goals with physicians, educate and empower families to carry over activities at home, coordinate with referral partners, and ensure continuity of care post-discharge through home exercise programs. Maintain patient documentation in an EMR, protect patient confidentiality, and participate in ongoing mentoring, training, and professional development.
Required Qualifications
- State license
- Current CPR certification
- A minimum of 1 yr. of experience preferred
