Job Summary
Provide pediatric physical therapy services in home and community settings, including evaluation, goal development, treatment planning, and caregiver coaching to support children with physical, neurological, cognitive, and social-emotional needs. Implement skilled interventions to maximize independence, educate families to carry over activities, coordinate with physicians and referral partners, ensure continuity of care post-discharge with home exercise programs, and document care in an EMR. Role may be part-time or full-time with a sign-on bonus.
