Job Summary
Physical Therapist (PT) will provide pediatric therapy in home and community settings, implementing evaluation-driven treatment plans to support child and family goals. Responsibilities include conducting evaluations, developing and executing treatment plans to improve physical, neurological, cognitive, and social-emotional functioning; educating and empowering caregivers to apply therapeutic activities at home; coordinating with referral partners; ensuring continuity of care post-discharge through home exercise programs; and documenting and safeguarding patient information in an EMR.
Required Qualifications
- State license
- Current CPR certification
- A minimum of 1 yr. of experience preferred
