SHR-Licensed Social Worker Honolulu - On Call Day Shift

Conduct psychosocial assessments, develop service plans, and coordinate care for patients and families while serving as a liaison between Kaiser Permanente and community agencies. Perform case management, advocacy, and resource identification to prevent psychosocial dysfunction and enhance personal functioning. Manage clinical record audits, implement quality improvement processes, and lead research through formal data analysis. Maintain educational records, participate in departmental committees, and orient new personnel. Requires a Hawaii license, BLS certification, and healthcare experience; on-call day shift rotation applies.
