SHH Registered Nurse PRN

Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary team reports. Plan, coordinate, implement, and evaluate patient treatment plans while making initial evaluations and re-evaluating nursing needs during each visit. Communicate significant findings, problems, and changes in health conditions or unsafe facility conditions to clinical supervisors, physicians, and other personnel. Revisit the plan of care in response to identified issues and notify relevant team members. Maintain patient and staff privacy and confidentiality pursuant to HIPAA. Requires current unrestricted state RN license, one year of clinical experience, and ability to travel 50% within coverage area. Associates or Bachelor's degree in nursing preferred.
