Job Summary
Assess resident needs, develop individualized service and treatment plans, and conduct regular check-ins. Connect residents to community resources, social supports, and essential services while coordinating follow-ups with providers. Maintain accurate documentation, resident records, and provide timely reports to supervisors. Perform crisis intervention as needed and collaborate with a multidisciplinary team to ensure compliance with program requirements.
Required Qualifications
- BS/BA Degree in a human services related discipline
- 3-5 years' experience working with applicable populations
- Experience working with unhoused populations
- 1-2 late days per week
- one weekend day (selected sites)
