Shelter Case Manager (Full-Time)

Job Summary

Conduct intake, assessment, and case planning for youth aged 18-24 to develop Individual Service Plans addressing housing, medical, and educational goals. Link clients to external services, coordinate across clinical and outreach departments, and facilitate employment and education partnerships through resume building and interview preparation. Monitor client progress, coach on goal implementation, and provide individual and group counseling using a trauma-informed, strength-based approach. Engage youth in community settings such as parks, job sites, and grocery stores, while maintaining weekly supervision and reporting statistics. Transport clients in company vans and attend off-site meetings, requiring a valid California driver's license. Work full-time, 40 hours per week, with some weekend availability.

Required Qualifications

  • At least 25 years old
  • Bachelor's Degree in Psychology, Sociology, Social Work or a related field
  • 2-3 years of professional experience working with at-risk/homeless youth, homeless services, child welfare or serving other at-risk groups
  • Prior Case Management Experience
  • An ability to build positive rapport with youth
  • Knowledge of the full range of public and social services in Alameda County and the Greater Bay Area for low-income, homeless, and foster youth
  • Excellent communication skills
  • On-point, timely, and professional correspondence
  • Written inter-agency coordination skills
  • Verbal inter-agency coordination skills
  • Scheduling appointments
  • Maintaining collaboration with multiple stakeholders and providers
  • A strength based approach
  • A trauma informed approach
  • Must be bilingual - English/Spanish
  • Valid California Driver's License
  • An acceptable driving record
  • Must be able to lift up to 40 lbs
