Job Summary
Transport the individual to medical appointments, activities, and daily outings while maintaining a safe, clean home environment. Assist with daily living skills and support individual goals through consistent care. Complete monthly paperwork and ensure all adults in the home meet background check, safety, and training requirements. This contracted Host Home position offers flexible home-based care with payments based on the level of care needed.
Required Qualifications
- All adults in the home must pass a Criminal Background Check
- Must be 21 years of age or older
- Must have experience providing support to individual(s) with disabilities
- Meet home/apartment safety and health requirements
- Complete pre-service training requirements
- Capable of communicating well in both verbal and written form
- Must have and maintain a valid driver's license and clean driving record
- Have good judgment
- Have a positive attitude

- Has experience working with people with disabilities
- Nursing/CNA background
- Teacher/Aide background
- Caregiver background
