Job Summary
Service Navigator provides individual mentoring, holistic service planning, and coordinated case management for probation-involved youth in Albany County. Responsibilities include maintaining progress notes and service plans, issuing incentive stipends as directed, making referrals and coordinating transportation for appointments, attending routine team meetings with youth, families, and Albany County Probation, and building positive relationships with youth and families. The role requires knowledge of community services in Albany County, strong record-keeping, cultural sensitivity, client confidentiality, and the ability to work on a hybrid schedule with about 50% community travel. Reports to the Assistant Director of Juvenile Justice for Prevention and collaborates with Probation and subcontracted staff to support youth and families.
Required Qualifications
- High School Diploma and two years relevant experience or Associate's Degree and one year relevant experience
