Job Summary
Non-medical Case Manager on Avenue 360’s Social Services team serving People Living With HIV (PLWH). Provides information, referrals, and linkage to Social Drivers of Health needs (medical, mental health, substance abuse, housing, housing financial assistance, food, etc.). Advocates for clients to reduce service gaps and helps develop independent living skills. Performs RWGA-approved assessments within five working days of intake and reassessments at six‐month intervals; conducts comprehensive assessments for mental health, substance abuse, and housing issues; refers clients to a Medical Case Manager when appropriate. Builds linkages to transportation resources, housing stability support, food assistance, and medical services; documents services in CPCDMS and other databases; ensures timely data entry for chart closure and billing. Compiles data for regular reports, participates in monthly supervision, and keeps abreast of new social work and medical treatment knowledge. Requires a bachelor’s degree (or equivalent experience) and one year of HIV-service experience; proficiency in Microsoft Office; CPCDMS/EHR experience desired; bilingual English/Spanish preferred; CAC certification due within 90 days of hire.
Required Qualifications
- Bachelor’s degree in social or behavioral sciences
- One year of paid work experience in the provision of services to individuals living with HIV
- Proficiency in Microsoft Office Suite; experience with CPCDMS and EHR database systems desirable
- Bilingual English and Spanish highly desired
- Certified Application Counselor (CAC) certification must be obtained within 90 days of hire
