Service Coordinator

Job Summary

Conduct intake assessments, develop individualized care plans, and coordinate referrals with community partners to ensure clients access person-centered services. Provide ongoing advocacy, crisis intervention, and support while maintaining accurate, confidential documentation in compliance with agency requirements. Deliver activity and floor coverage, supervise social work students, and represent the organization's mission in all interactions. Participate in team meetings, committees, and training seminars to promote service integration and professional development. Occasional weekend work may be required; background check is mandatory.

Required Qualifications

  • Occasional after-hours or weekend work
  • Strong interpersonal and communication skills with a client-centered, trauma-informed approach
  • Ability to establish and maintain effective relationships with clients, co-workers, and community partners
  • Excellent organizational and time management skills, with attention to detail and accountability for follow-through
  • Commitment to diversity, equity, inclusion, and cultural humility
  • Familiarity with client and personnel record-keeping systems (e.g., SetWorks, Paylocity)
  • Valid driver's license
  • High School Diploma or equivalent
  • Must be able to pass a background check
  • Occasional travel to off-site locations, meetings, or events

Desired Qualifications

  • Associate or bachelor's degree in social work, human services, psychology, or related field
  • Experience working in a direct support role
  • Experience working in a nonprofit or mission-driven organization
  • Familiarity with local community resources, social service systems, and public benefits
