Job Summary
Service Coordinator I coordinates programs and services to help participants maintain quality of life and age in place, serving as a liaison to community agencies, documenting services, organizing educational events, and connecting participants with health services and community resources. The role involves outreach and enrollment activities, individual assessments, referrals, care coordination, documentation across systems (including HUD and funder reporting), pursuit of partnerships to provide health services and screenings, and maintaining participant records and plans with attention to compliance, privacy (HIPAA), and quality metrics. Strong communication, organization, and ability to work both autonomously and as part of a team are required, with a focus on person-centered service delivery and adherence to program requirements. Travel may be required as directed by contract and department requests, and the position operates in a housing/elderly services setting.
Required Qualifications
- High School Diploma or GED required
- A bachelor’s degree in social work or a related field is preferred
- Two or more years of experience in a social service delivery with elderly, people with disabilities, and/or family population
- Demonstrated working knowledge of supportive services and other resources in the area served
- Demonstrated ability to advocate, organize, problem-solve, and provide results for the participants served
- Good communication, comprehension and interpersonal skills
- Working knowledge of Windows, Microsoft Office Suite
