Job Summary
Service Coordinator to provide comprehensive case management for children ages 3 to 6 and their families, collaborating with Regional Center staff to coordinate medical, behavioral health, educational, vocational, recreational, and adaptive services; develop and implement individualized Family Service Plans (IFSP); document plans and progress; initiate purchase of service (POS) for clients; conduct in-person meetings with families and agencies; contribute to multidisciplinary teams and ensure timely reporting; Spanish bilingual preferred; requires a Bachelor's degree in a related field and at least one year of relevant experience.
Required Qualifications
- Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or a related field
- At least one year of experience working with individuals with developmental disabilities or a related field
- Master's degree in a related field can be substituted for the required experience
- Bilingual in Spanish is preferred
