Service Coordinator-SPOE

Job Summary
Conduct initial service coordination during the referral and evaluation process for children and families in early intervention. Obtain System Point of Entry (SPOE) data, maintain accurate child records, and ensure all documentation meets state eligibility criteria and agency standards. Facilitate communication with families, medical professionals, and NJ DCP&P personnel to promote coordinated care while explaining rights, entitlements, and funding differences between Part C and Part B. Coordinate evaluations, manage county spreadsheets, and oversee transitions to appropriate services or exit pathways. Interact with families facing mental illness, abuse, or chronic illness to support their enrollment.
Required Qualifications
- Bachelor's degree or higher from an accredited institution in a related health, child development, social work, human service, or education field
- Minimum of one year of documented professional experience working with children & families in a counseling, supporting, or case management role
- Documented professional experience with individual birth to five years of age and their families
- Knowledge and experience relating to the field of non-profit, maternal child health and education
- Bilingual (English/Spanish)
- Computer proficiency with Microsoft programs including Windows and Office
- Excel & Access
- Strong interpersonal & oral/written communication skills
- Valid Driver's license
- Ability to provide proof of ongoing automobile insurance coverage
- Ability to attend meetings at external locations
