Job Summary
Service Coordinator for SPOE role responsible for guiding children and families through referral, evaluation, and eligibility determination in early intervention. Primary duties include initiating SPOE referrals, conducting professional family interviews via phone, explaining rights and funding differences between Part C and Part B/IDEA, maintaining confidentiality and accurate documentation, coordinating with medical and NJ DCP&P partners, data entry using state referral forms, managing child records, ensuring timelines per DOH criteria, and coordinating transitions to appropriate SCU. Requires collaboration across agencies, strong communication skills (bilingual in English/Spanish preferred), and reliable documentation of eligibility decisions and resources.
Required Qualifications
- Bachelor's degree or higher in a related health, child development, social work, human service, or education field
- Minimum of one year of professional experience working with children & families in counseling, supporting, or case management
- Bilingual English/Spanish
- Computer proficiency with Microsoft Office; Excel & Access preferred
- Valid Driver’s license with ability to provide proof of ongoing automobile insurance
- Willingness to travel locally and statewide
