Service Coordinator

Job Summary
Coordinate services for individuals with developmental disabilities by consulting with managers, specialists, and vendors to facilitate medical, behavioral, educational, and vocational planning. Conduct in-person meetings with clients, families, and agencies to develop and implement individual Program Plans, while explaining available resources and documenting progress through inter-disciplinary notes and mandated reports. Initiate purchases of service for ineligible clients and present cases at specialized committees. This community-based role requires 60% office time and 40% community presence, including home visits and driving to appointments. A valid driver's license and reliable transportation are essential.
Required Qualifications
- Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or a related field
- At least one year of experience working with individuals with developmental disabilities or a related field
- Master's degree in a related field can be substituted for the required experience
- Knowledge of developmental disabilities and community resources
- Strong interpersonal and communication skills (verbal and in writing)
- Experience in developing and implementing individualized service plans and meeting strict document timelines
- Knowledge of behavior intervention techniques and best practices
- Ability to independently plan and schedule work
- Ability to develop individualized support plans and adapt strategies as needed
- Strong organizational skills and attention to detail for documentation and case management
- Ability to conduct in-person meetings and facilitate collaboration among various stakeholders
- Excellent computer skills in MS Office, including Word and Outlook
- Ability to multi-task, prioritize assignments, and meet deadlines with conflicting priorities
- Ability to provide a compassionate, culturally sensitive approach to working with diverse populations
- Service coordination is a community-based position that requires meeting with individuals served and their families in their home, in the community, or in our office location
- Frequent telephone, virtual, and out-of-office meetings are required
- A valid driver's license, reliable transportation and minimum liability insurance coverage is required
- Work 60% of the time in an office environment and 40% in the community at meetings or home visits
- Ability to drive to and from meetings or home visits and walk from car to and from appointments
- Occasional walking and standing to attend and participate in office meetings and trainings
- Ability to sit for extended periods and operate a computer for report writing
- Clear verbal communication in person and by phone
- Occasional lifting, pushing, or pulling of items up to 25 pounds
- Adequate visual acuity to read documents, use computer monitor for prolonged periods, and review detailed information
