Job Summary
Evaluate residents' needs to identify services enhancing their quality of work and independence. Establish connections with local providers to facilitate access to social, health, and wellness resources. Create and sustain educational programs tailored to resident needs while maintaining a comprehensive directory of community services. Act as a resident advocate, empowering individuals to advocate for themselves and liaising between residents and external agencies. Build positive relationships with community agencies, networking with service providers to identify new services. Foster effective communication between residents and providers, ensuring an open environment. Track and monitor resident referrals to external agencies, ensuring follow-up and service delivery. Maintain accurate daily and monthly reports documenting service coordination and interactions. Provide information, referrals, and support for residents requiring crisis intervention and conflict resolution. Work independently with minimal supervision to support the mission of Round House Manor, a 49-unit older adult residential community in Kaukauna, WI.
Required Qualifications
- Strong understanding of available supportive services and community resources for older adults
- Proven ability to advocate for residents' needs and resolve issues
- Commitment to maintaining high ethical standards and client confidentiality
- Ability to work effectively with residents, staff, community partners, and service providers
- Capable of working independently and with minimal supervision
- Excellent verbal and written communication skills
- Strong organizational and time management skills
- Proficiency in MS Office Suite, including Word, Excel, PowerPoint, and Outlook
Desired Qualifications
- Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field
- Experience working with diverse groups of seniors and disabled individuals
