Job Summary
Evaluate residents' needs to identify services enhancing quality of life and promote independence. Establish connections with local providers to facilitate access to social, health, and wellness resources. Create and sustain educational programs tailored to residents' needs while maintaining a comprehensive directory of community services. Act as a resident advocate, empowering individuals and liaising between residents, management, and external agencies for social services and health benefits. Provide information, referrals, and support for crisis intervention and conflict resolution. Track and monitor resident referrals to ensure follow-up and service delivery. Maintain accurate daily and monthly reports documenting service coordination and resident interactions. Work part-time, 32 hours per week at Parmly Lakeview in Chisago City, MN.
Required Qualifications
- Strong understanding of available supportive services and community resources for older adults
- Proven ability to advocate for residents' needs and resolve issues
- Commitment to maintaining high ethical standards and client confidentiality
- Ability to work effectively with residents, staff, community partners, and service providers
- Capable of working independently and with minimal supervision
- Excellent verbal and written communication skills
- Strong organizational and time management skills
- Proficiency in MS Office Suite, including Word, Excel, PowerPoint, and Outlook
Desired Qualifications
- Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field
- Experience working with diverse groups of seniors and disabled individuals
