Job Summary
Service Coordinator position supporting individuals with disabilities through coordinated access to medical, social, vocational, financial, educational, and other services. Reports to the IDD Program Administrator of Service Coordination within the IDD division. Responsibilities include managing caseload paperwork, communicating with families and stakeholders, staff collaboration, crisis resources when needed, documentation in the electronic health record, and maintaining trauma-informed, person-centered interactions. Targets emphasize engagement, timely notes, and chart reviews. Requires independent and collaborative work, strong organizational skills, bilingual (English/Spanish) preferred, and reliable transportation.
Required Qualifications
- A bachelor’s or advanced degree from an accredited college or university preferred OR an associate degree in a social, behavioral, human service, or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice, AND one year of paid or unpaid experience with individuals with intellectual or developmental disabilities.
- OR a high school diploma or certificate recognized by a state as the equivalent of a high school diploma, AND two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
- Must have reliable transportation and a valid driver’s license.
- Knowledge of LifePath Systems services and ability to explain them to individuals served, family members, and community partners appropriately.
