Job Summary
Coordinate and deliver mental health case management services on the Crestwood San Jose campus. Assess clients to identify treatment needs, coordinate interdisciplinary team meetings and discharge planning, and implement the overall treatment plan in collaboration with nursing staff. Requires two years of experience in a mental health setting or a bachelor’s degree substituted for experience; knowledge of DSM-5, psychosocial rehabilitation, recovery principles, and regulatory standards. Provides full-time salary in the $25–$27/hour range with comprehensive benefits and growth opportunities.
Required Qualifications
- Two (2) years of full-time experience or part-time equivalent in a mental health setting.
- A bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
