Job Summary
Assess clients to identify individual problem areas necessitating treatment and complete assessments. Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. Coordinate overall treatment plan implementation for the client on the unit, working closely with nursing staff to ensure continuity of treatment efforts. This full-time PM role is located in Santa Barbara, California, with a pay range of $24.20–$25.68 USD. Candidates require two years of experience in a mental health setting or a relevant bachelor's degree.
Required Qualifications
- Two (2) years of full-time experience or part-time equivalent in a mental health setting
- A bachelor's degree with a major in psychology, social work, or behavioral sciences
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles
- Familiarity with regulatory standards
- Employment is contingent upon successful completion of a background investigation including criminal history and identity check
- Crestwood is required to participate in the E-Verify program
