Job Summary
Assess clients to identify individual problem areas necessitating treatment and complete assessments. Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. Coordinate the overall treatment plan implementation for the client on the unit, working closely with nursing staff to ensure continuity of treatment efforts. This role supports guests at Freise Hope House, a Crisis Residential Treatment Program, using recovery-based tools such as Dialectical Behavior Therapy and Trauma-Informed Approaches.
Required Qualifications
- A minimum of 2 years in a healthcare setting, at least one of which must have been in a mental health setting
- Must be registered with the BBS as AMFT, ASW, or LPCC
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles
- Familiarity with regulatory standards
- Employment is contingent upon successful completion of a background investigation including criminal history and identity check
- Crestwood is required to participate in the E-Verify program
