Job Summary
Assessment and verification of service coordination needs, followed by service planning, referral, monitoring of service provision, and advocacy for consumers. Manage after-hours crisis services and document progress while maintaining frequent contact with consumers, family members, and service providers. Accommodate consumer and family needs by working a flexible schedule that includes after-hours, holidays, and weekends. Bachelor's degree or relevant associate degree with experience in intellectual and developmental disabilities required; bilingual Spanish and English a plus. Full-time employment includes loan forgiveness, health insurance sharing, and 13 paid holidays.
Required Qualifications
- Bachelor's degree or advanced degree from an accredited college or university (any field)
- Associate degree in a social, behavioral, human service, or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice
- High School diploma or a certificate recognized by a state as the equivalent of a high school diploma
- Two years of paid or unpaid experience with individuals with intellectual or developmental disabilities
Desired Qualifications
- One or more years of work experience serving individuals with intellectual and developmental disabilities
- Working computer skills
- Bilingual in Spanish & English
