Job Summary
Conduct service planning, monitoring, and assessment for consumers with intellectual and developmental disabilities to identify needs, goals, and recommendations for interventions. Collaborate with service providers and community agencies to link consumers with outcome-focused services while managing a caseload of up to 45 individuals utilizing HCS and TXHML waiver funding streams. Make regular contacts with assigned consumers, complete annual person-directed plans, and document service delivery accurately. Requires flexibility in scheduling, significant travel within Atascosa, Frio, and Wilson Counties, and meeting monthly productivity standards. This role is housed at the Maverick CRU Admin office in Eagle Pass, TX, under the IDD Waiver Programs Director.
Required Qualifications
- Bachelor's degree in social work, psychology, human services, or a related field
- Knowledge of community resources
- Strong organizational and communication skills
- Requires flexibility in scheduling
- Significant travel within the service area
- Self-motivated
- Energetic
- Manage a caseload not to exceed forty-five (45) individuals
- Make contacts with assigned consumers in accordance to their service coordination assessment
- Complete an annual person-directed plan (PDP) for each person on their caseload
- Complete timely and accurate documentation of the delivery of service coordination services
- Meet set productivity standards on a monthly basis
- Housed at the Maverick CRU Admin office in Eagle Pass, TX
- Directly supervised by the IDD Waiver Programs Director
Desired Qualifications
- Experience working with individuals with intellectual and developmental disabilities
- Experience with person-centered planning and trauma-informed care
- Bilingual abilities or experience working with diverse populations
