Job Summary
Service Coordinator in the Intensive Case Management Unit (Sacramento) providing case management and advocacy for individuals with developmental disabilities, coordinating supports (transportation, vocational training, day programming, education, mental health services, medical care, residential placement, independent living training), developing and monitoring Individual Program Plans, liaising with DDS conservatorship, and completing required forms and reports; collaborates with DDS conservatorship liaison and vendors, rotates duties with the unit, and supports co-workers on special projects.
Required Qualifications
- Bachelor’s or Master’s degree in Social Work or a related field
- Excellent written, verbal, and interpersonal communications skills
- Strong time management and organization skills
- One year of professional experience in social work or a related field (preferably in developmental disabilities)
