Job Summary
Conduct case management and linkage services for enrolled members utilizing project-based and scattered site housing in Los Angeles, Pasadena, and Santa Monica. Evaluate participants to identify social, emotional, and economic factors interfering with stability; obtain consents and authorizations as needed. Assist members with recertification, housing access, move-in processes, and transport to unit searchers, lease signings, and healthcare providers. Collaborate on measurable service plans, budgeting, household management, and linkage to benefits, medical, mental health, and substance abuse resources. Document services within 72 hours using G.I.R.P. format and maintain accurate files in agency databases. Liaise with internal and external providers to formulate strength-based strategies focusing on housing retention and wellness.
Required Qualifications
- Minimum of 2 years' experience working with people who experience homelessness and or are diagnosed with a mental-illness
- Social Work/Mental Health related Bachelor's degree
- minimum of 1 years' experience working with people who experience homelessness and or who are diagnosed with mental-illness in a community or housed setting
- Ability to work as a part of a team
- Knowledge of issues facing program members, (e.g. health, substance abuse, mental health, physical health, domestic abuse, child welfare, resources for undocumented person)
- excellent written and verbal communication skills
- must be able to perform extensive charting, electronic data entry and documentation
- Microsoft 365 Office is essential
- Must have own transportation
- current California driver's license
- insurance with good driving record at all times during employment

- Familiarity with and dedication to the Housing First and Harm Reduction models
