Job Summary
Collaborate with clients and support teams to develop and update individualized service plans based on personal goals. Coordinate and monitor services with paid providers and community resources to ensure they meet needs, are authorized, and follow the plan. Maintain open communication with support teams to address concerns promptly and ensure all work complies with program regulations, documentation, and billing requirements. Travel within the assigned region to meet participants in compliance with service plans. This hybrid role requires living in or traveling to Pennsylvania for home visits and involves frequent exposure to outdoor elements and varying weather conditions. Employees must maintain reliable high-speed internet for remote work and complete background checks including PA State Police, FBI, and child abuse clearances.
Required Qualifications
- 4-year degree
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- At least three (2) years of professional experience in case management for people with physical disabilities
- Three (2) years of experience working with individuals with Autism
- Must live in PA or be willing to travel to PA to complete home visits
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check

Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred
