Job Summary
Collaborate with clients and support teams to develop and update individualized service plans based on personal goals. Coordinate and monitor services with paid providers and community resources to ensure they meet individual needs and follow the plan. Maintain open communication to address concerns promptly and ensure all work complies with program regulations, documentation, and billing requirements. Travel within the assigned region to meet participants in compliance with service plans. This hybrid position requires reliable high-speed internet access and may involve driving in varying weather conditions.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- At least two (2) years of professional experience in case management for people with physical disabilities
- Two (2) years of experience working with individuals with Autism
- Must live in PA or be willing to travel to PA to complete home visits
- Must have reliable high-speed internet access at home
- Must be able to work effectively as part of an interdisciplinary team
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check
Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS)
