Job Summary
Collaborate with clients and support teams to develop, update, and manage personalized service plans based on individual goals and priorities. Coordinate and monitor services with paid providers and community resources to ensure they meet needs, are authorized, and follow the plan. Maintain open communication to address concerns promptly and ensure all work complies with program regulations, documentation, and billing requirements. Travel within the assigned region to meet participants while maintaining reliable high-speed internet for remote work. This hybrid role requires living in or traveling to Pennsylvania and involves working with individuals with disabilities.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- Must live in PA or be willing to travel to PA to complete home visits
- Must have reliable high-speed internet at home
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check
Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred
