Job Summary
Collaborate with clients and support teams to develop and update individualized service plans based on personal goals. Coordinate and monitor services with paid providers and community resources to ensure they meet needs and follow the plan. Maintain open communication to address concerns promptly and ensure all work complies with program regulations, documentation, and billing requirements. Travel within the assigned region to meet participants in compliance with service plans. This hybrid position requires reliable high-speed internet access and the ability to drive in varying weather conditions. Candidates must reside in Pennsylvania or be willing to travel for home visits. The role involves working with individuals with disabilities to enhance their quality of life and independence.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- Must live in PA or be willing to travel to PA to complete home visits
- Must have reliable high-speed internet at home
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check
Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred
