Job Summary
Develop individualized service plans with clients and support teams based on personal goals, then regularly update these plans to reflect life changes and current support needs. Coordinate and monitor services with paid providers and community resources to ensure they meet individual needs, are authorized, and are implemented correctly. Maintain open communication with support teams to address concerns promptly while ensuring all work complies with program regulations, documentation standards, and billing requirements. Travel within the assigned region to meet participants in compliance with service plans. This hybrid role requires reliable high-speed internet for remote work and may involve occasional driving in poor weather conditions.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- At least three (2) years of professional experience in case management for people with physical disabilities
- Three (2) years of experience working with individuals with Autism
- Must live in PA or be willing to travel to PA to complete home visits
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check
Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred
