Job Summary
Collaborate with clients and support teams to develop and update personalized service plans based on individual goals. Coordinate and monitor services with paid providers and community resources to ensure they meet needs and follow the plan. Maintain open communication to address concerns promptly while ensuring all work complies with program regulations and documentation standards. Travel within the assigned region to meet participants in compliance with individualized service plans. This hybrid role requires living in Pennsylvania or willingness to travel for home visits, with mandatory background checks including PA State Police, FBI, and child abuse clearances.
Required Qualifications
- 4-year degree
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
- At least three (2) years of professional experience in case management for people with physical disabilities
- Three (2) years of experience working with individuals with Autism
- Must live in PA or be willing to travel to PA to complete home visits
- PA State Police Background Check (e-PATCH)
- Child Abuse Clearance
- FBI Background Check
- MA Fraud Check
- Social Security Verification
- DMV Check

Desired Qualifications
- Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred
