Job Summary
Early Intervention Service Coordinator responsibilities include coordinating services for infants and their families, providing case management and advocacy, developing Individual Family Service Plans (IFSP) with families and collaborating agencies, conducting psychosocial and developmental assessments in the home, determining ongoing eligibility for services up to age 3, completing required forms and reports, rotating on-call duties, and offering emergency on-call support as needed. Requires strong written/verbal/interpersonal communications, time management and organizational skills, and the ability to drive in California.
Required Qualifications
- Master’s or Bachelor's degree in Social Work or a related field
- valid California driver’s license and automobile liability insurance
- 1 year professional experience in social work or a related field (preferably in developmental disabilities)
- Spanish speakers preferred
