Job Summary
Clinical Service Coordinator will conduct assessments of physical health, behavioral health, social services, and long-term supports needs for members, identify high-risk individuals, and help develop and implement person-centered service plans. Works as part of an interdisciplinary team to coordinate care, obtain clinical data, and coordinate services across settings. Role involves travel to members' homes or other locations (approximately 50% travel) and 50% remote work. Focused on behavioral health needs, with licensure such as RN/LSW/LCSW/LMSW required. Requires 2+ years of relevant experience; schedule is Monday–Friday, 8am–5pm; benefits include health/dental/vision, 401K, holidays, PTO, and mileage reimbursement.
Required Qualifications
- Current, unrestricted RN, LSW, LCSW, or LMSW license in applicable state(s) required
- Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role
- State-specified certification based on state law and/or contract may be required
- Strong communication and interpersonal skills
- Ability to work effectively as part of an interdisciplinary team
- Compassionate and patient-centered approach to care
