Job Summary
Service Coordinator Clinician that conducts comprehensive clinical assessments of members with chronic illnesses, co-morbidities, and disabilities, identifies needs across physical health, behavioral health, social services, and long-term supports, and collaborates with an interdisciplinary team to develop and implement person-centered service plans. Role involves 50% travel to members’ homes or other sites and 50% remote office work, focusing on behavioral health needs and coordination of care within waiver or alternative care programs.
Required Qualifications
- Current, unrestricted RN, LSW, LCSW, or LMSW license in applicable state(s) required.
- Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience that would provide an equivalent background.
- State-specified certification based on state law and/or contract may be required.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary team.
- Compassionate and patient-centered approach to care.
