Job Summary

Manage an assigned caseload of individuals with developmental disabilities to provide service coordination, advocacy, and case plan development consistent with State and Federal law. Coordinate vendored services, monitor health and safety, and serve as a liaison to facilities while conducting regular home and community visits. Prioritize urgent needs, document activities including Special Incident Reports and Title 19 notes, and lead Individualized Family Service Plans (IFSP/IPP) meetings for assigned age groups. Collaborate with clinical services, Department of Social Services, and foster care systems to ensure access to public and generic resources. Execute Officer of the Day duties, including triage, problem-solving, and responding to community inquiries. Requires a valid driver's license and ability to travel within the six-county region.

Required Qualifications

  • Bachelor's degree in human services related filed include but not limited to social work, psychology, public administration, and sociology
  • A minimum of two years of work experience including exposure to individuals with developmental disabilities and/or direct support, or case management experience
  • Advanced knowledge and understanding of developmental disabilities and related conditions faced by CVRC individuals
  • A high level of discretion and independent judgment
  • Ability to establish and maintain comfortable and effective relationships with other
  • Ability to communicate effectively, verbally and in writing
  • Ability to prioritize varied work assignments and complete documentation within required timelines
  • Ability to use computer and software for documentation, management information systems, e-mail, and other required computerized functions
  • Must demonstrate initiative, self-motivation, sound independent judgment, flexibility, strong interpersonal and excellent written and oral communication skills
  • Must have a valid CA driver's license
  • Must have a reliable vehicle
  • Must have vehicle insurance
  • Must be willing and able to drive for work on a regular basis
  • Must have the ability to travel within CVRC's six-county-wide region as assigned or required

Desired Qualifications

  • Knowledge of generic public resources and developmental disabilities desirable
  • Bilingual preferred
