Job Summary
Service Coordinator for children with developmental disabilities (ages 6-17) based in the Sacramento area. Responsibilities include participation in developing and monitoring the Individual Program Plan (IPP); assisting clients and families in obtaining and maintaining supports and services; advocacy with community agencies; vendor coordination, referrals, and completion of required forms/documents; rotating on-call duties; and assisting teammates with special projects. Requires a Master’s or Bachelor’s degree in Social Work or a related field; valid CA driver license and auto liability insurance; strong written/verbal/interpersonal skills; excellent time management and organization; Spanish speakers preferred.
