Job Summary

As a Service Coordinator, coordinate person-centered planning and ongoing case management for individuals with intellectual and developmental disabilities (I/DD) in Grants Pass, OR. Responsibilities include participating in Needs Assessments (ONA), developing and monitoring Individual Support Plans (ISPs), visiting clients in their homes or community settings to ensure health and safety, addressing critical incidents, facilitating service choices, and collaborating with brokerages, providers, schools, and medical teams to ensure compliance with state regulations and timelines. You will document thoroughly, maintain professional boundaries, and adhere to mandatory reporting requirements. A strong foundation in ODDS programs, rules, timelines, and documentation standards, along with trauma-informed practices and excellent organizational and communication skills, is essential to advocate effectively for clients and empower independence while delivering inclusive, strength-based services.

Required Qualifications

  • Bachelor's degree in behavioral science, social science, or a closely related field
  • Bachelor's degree in any field and one year of human services related experience
  • Associate's degree in a behavioral science, social science, or closely related field and two years of human services related experience
  • Three years of human services related experience
  • Valid driver's license
