Job Summary
Service Coordinator/Case Manager in Coos County coordinates with individuals, families, and providers to fulfill ODDS tasks such as Needs Assessments (ONA) and development/monitoring of Individual Support Plans (ISPs). Responsibilities include visiting clients in homes or community settings to ensure health and safety, addressing critical incidents, facilitating choices in services, and ensuring services are compliant with state regulations and timelines. Collaborates with brokerages, providers, schools, and medical teams; maintains meticulous documentation; advocates for person-centered planning and trauma-informed practices to empower individuals with intellectual and developmental disabilities to live as independently as possible.
Required Qualifications
- Bachelor's degree in behavioral science, social science, or closely related field
- Bachelor's degree in any field with one year of human services related experience
- Associate's degree in behavioral science, social science, or closely related field with two years of human services related experience
- Three years of human services related experience
- Valid driver's license
