Service Coordinator-Bilingual

Job Summary
Coordinating IFSP development/evaluation and maintaining regular contact with families to ensure timely service delivery; document case notes; inform parents about advocacy services; develop transitional/discharge plans; attend trainings and meetings; ensure timely PPS entries and audit compliance; support Birth-to-Three in natural environments; assist Program Manager with file audits; serve as a center ambassador; perform in-home visits across Milwaukee County; bilingual (Spanish) communication required; driving required for 33% of duties.
Required Qualifications
- Bachelor’s degree in a Social Work, Human Services, Child Development.
Additional Requirements
- Must be able to complete home visits in-person anywhere in Milwaukee County on a daily basis.
- Fluency in reading, writing, and speaking Spanish is required.
- Must have a valid Wisconsin driver’s license and may not have more than three current minor driving violations and/or one current at-fault accident on your record. Auto insurance must include $100,000/$300,000 bodily injury and $100,000 property damage.
