Job Summary
Coordinate potential services and providers to meet individual needs determined in IFSPs, developing and reviewing plans with families and multidisciplinary teams. Counsel individuals and families to prioritize services, organize new requests for eligibility determination, and monitor health and safety issues while ensuring adherence to DIDD rules and Part C laws. Serve as a liaison to communicate with stakeholders, interpret documents, request data updates, and develop collaborative relationships with providers and community partners. Document case notes and data in UNICORN, problem-solve complex client issues, and implement creative avenues to increase revenue for direct services. Work a 4-day, 40-hour week Monday through Thursday with flexibility for remote or in-person collaboration in Aurora, CO.
Required Qualifications
- Bachelor's degree
- Five (5) years' experience in the field of developmental disabilities
- A combination of education and experience appropriate to the requirements of the position
- Personal vehicle in good operating condition for use during work as appropriate
- Valid Colorado motor vehicle license
- Proof of motor vehicle insurance
- Ability to meet our driving requirements - no suspensions/ no more than two tickets in the past three years

Desired Qualifications
