Job Summary
Coordinate potential services and providers to meet individual needs determined in the IFSP, while developing and reviewing IFSP/FSP documents with families and multidisciplinary teams. Schedule and facilitate meetings, counsel individuals and families on service priorities, and organize new service requests for eligibility determination. Monitor health and safety issues, facilitate referrals to other agencies, and ensure adherence to DIDD rules, Part C law, and agency policies. Serve as a liaison to communicate with stakeholders, interpret documents, and request data updates. Actively develop collaborative relationships with providers, Child Find staff, and community partners, and problem-solve complex client issues. Document case notes and data entry in UNICORN as outlined by department expectations.
Required Qualifications
- Bachelor's degree
- Five (5) years' experience in the field of developmental disabilities
- A combination of education and experience appropriate to the requirements of the position
- Personal vehicle in good operating condition for use during work as appropriate
- Valid Colorado motor vehicle license
- Proof of motor vehicle insurance
- Ability to meet driving requirements - no suspensions/ no more than two tickets in the past three years

Desired Qualifications
- Experience with Child Find and/or Part C laws and regulations
