Job Summary
Serves as the primary Service Coordinator for the consumer on the caseload in the General Revenue and Community First Choice programs. Responsible for developing a Person Directed Plan (PDP) based on the needs and desires of the consumer and for ongoing evaluation of the plan. Facilitates planning meetings to identify consumer needs, conducts service planning, monitors plan progress, and addresses crisis prevention and documentation within required timeframes. Acts as a liaison between clients, families, and internal service providers; may supervise others. May drive to five service counties to meet with consumers, attend planning meetings, and participate in face-to-face contacts. Performs complex, journey-level social services work and ensures timely documentation in the Electronic Health Record (EHR). Requires on-site work for the first six months with potential approval to work remote thereafter; must report to office for scheduled meetings and events. Strong skills in planning, assessment, communication, and collaboration with schools and community providers; proficiency with Microsoft Word and Excel. ADA-compliant, able to manage travel, multitask, and adapt to changing priorities in a dynamic environment.
Required Qualifications
- Bachelors or advanced degree in psychology, social work or related social services area, OR Associates degree in a social, behavioral, human services or health related field OR High School diploma with two years of experience with individuals with intellectual or developmental disabilities
