Service Coordination Manager

Conduct client care coordination by collaborating with the Clinical Manager to act as an internal case manager and advocate within the Habilitation program. Implement and evaluate individual goals through Person-Centered Planning processes while making regular visits to monitor service implementation and progress. Supervise Habilitation Technicians according to policy and state regulations, maintaining ongoing communication with Case Managers and Care Coordinators. Perform client-specific orientations as directed and travel locally for intake conversations. Requires a Bachelor's degree, experience with the I/DD population, and prior supervisory experience.
