Job Summary
Support adults to live independently in their own homes by providing personal care, medication assistance, meal preparation, and domestic tasks while acting as a Key Worker for assessments, care planning, and therapeutic activities. Guide and supervise Support Workers to ensure values-based care is delivered to the highest standard, maintaining accurate and up-to-date support plans. Work with individuals across Cheshire West and Chester on a person-centred basis, adapting daily to diverse needs and responding effectively to individual situations. Requires previous Support Worker experience, car access, and the ability to work various shifts including weekends.
