Job Summary
Meet patients and families to assess long-term care needs and provide consultative guidance on living arrangements and safety concerns. Serve as the regional point of contact for onboarding new patients and maintain a thorough knowledge of local senior care resources including assisted living facilities, home care agencies, and community programs. Act as a liaison to ensure seamless access and coordination, while educating community professionals and providers through outreach and training. Support care transitions by collaborating with transitional care providers and interdisciplinary teams, and assist families in accessing care management, advance care planning, and hospice services. Track and document activities for billable Medicare services and participate in community events to promote SSA services. Collaborate with the Director of Development to support market expansion through strategic relationship building and referral development. This full-time, 8-hour role requires daily travel to patients' homes and facilities in the [location] area.
Required Qualifications
- Valid driver's license
- Reliable transportation
- Must be a licensed driver
- Automobile insured in accordance with state and/or organizational requirements
- Automobile in good working order
- Ability to work flexible hours
- Ability to travel between offices, facilities, etc.

- Bachelor's Degree in Social Work, Human Services, Healthcare Administration, or related field
