Job Summary
Meet patients and families to assess long-term care needs and social support challenges, providing consultative guidance on living arrangements and safety concerns. Serve as the regional point of contact for onboarding new patients and act as a liaison between individuals and local senior care resources, including assisted living facilities, home care agencies, and community programs. Educate community professionals and family caregivers through outreach and training while supporting care transitions with Transitional Care Providers. Track and document activities for billable Medicare services and participate in community events to promote SSA services. Collaborate with the Director of Development to support market expansion through strategic relationship building and referral development. This full-time, salary-based 8-hour role (8:00AM-5:00PM, Monday-Friday) requires a valid driver's license and daily travel to patients' homes and facilities in the designated area.
Required Qualifications
- Valid driver's license
- Reliable transportation
- Must be a licensed driver
- Automobile that is insured in accordance with state and/or organizational requirements
- Automobile in good working order
- Ability to work flexible hours
- Ability to travel between offices, facilities, etc.
Desired Qualifications
- Bachelor's Degree in Social Work, Human Services, Healthcare Administration, or related field
- Prior experience in case management, senior services, or healthcare navigation
- Strong interpersonal and communication skills
