Job Summary
Meet patients and families to assess long-term care needs and social support challenges, providing consultative guidance on living arrangements and safety. Serve as the regional point of contact for onboarding new patients and maintain thorough knowledge of local senior care resources including assisted living, home care, and faith-based programs. Act as a liaison to ensure seamless access to these resources, educate community professionals through outreach and training, and support care transitions with interdisciplinary teams. Track and document activities for billable Medicare services while collaborating with the Director of Development to support market expansion. This full-time, 8-hour position (8:00AM-5:00PM, Monday-Friday) requires daily travel to the Marietta area.
Required Qualifications
- Valid driver's license
- Reliable transportation
- Must be a licensed driver
- Automobile insured in accordance with state and/or organizational requirements
- Automobile in good working order
- Ability to work flexible hours
- Ability to travel between offices, facilities, etc.
Desired Qualifications
- Bachelor's Degree in Social Work, Human Services, Healthcare Administration, or related field
- Prior experience in case management, senior services, or healthcare navigation
