Job Summary
Assess long-term care needs and social support challenges for patients and families in the designated area. Provide consultative guidance on living arrangements and safety concerns while serving as the regional point of contact for onboarding new patients. Maintain knowledge of local senior care resources including assisted living, home care agencies, and community programs to ensure seamless access and coordination. Educate community professionals and family caregivers through outreach and training, supporting care transitions with Transitional Care Providers. Track billable Medicare services and document activities per billing structures. Participate in community events and collaborate with the Director of Development to support market expansion. Full-time, 8-hour schedule (8:00AM-5:00PM, Monday-Friday) with daily travel to patients' homes and facilities. Bachelor's degree in Social Work, Human Services, or Healthcare Administration preferred; prior case management experience strongly preferred. Valid driver's license and reliable transportation required.
