Senior Solutions Advisor

Job Summary
Senior Solutions Advisor travels to patients' homes and facilities in the Walterboro/Orangeburg area to assess long-term care needs and social support challenges; provides consultative guidance on living arrangements and safety; serves as regional point of contact for onboarding new patients; maintains knowledge of local senior care resources; liaises between patients, families, and resources to ensure access and coordination; educates community professionals, families, and providers through outreach and training; supports care transitions with Transitional Care Providers and interdisciplinary teams; documents activities for billing (e.g., CCM, ACP, etc.); participates in community events and referrals to support market expansion.
Required Qualifications
- Bachelor’s Degree in Social Work, Human Services, Healthcare Administration, or related field preferred
- Prior experience in case management, senior services, or healthcare navigation strongly preferred
- Valid driver’s license and reliable transportation required
- Ability to travel between offices, facilities, etc.
- Strong interpersonal and communication skills
- Self-starter with time management and follow-through skills
- Proficient in basic computer use
- Ability to foster cooperative work environment
- Team player with ability to manage multiple responsibilities and sound judgment
- Must be a licensed driver with insured automobile in good working order
